6.1.4.1.2 Minimal information requirements

It is not necessary to receive a completed claim form to determine Immediate Support payments. However, the following information must be obtained before any decision can be made:

Information Details required
Worker details

The personal particulars of the deceased worker, including:

  • full name
  • date of birth
  • address.
Death of worker

Particulars as to the death of the worker, including:

  • date of death
  • place of death
  • circumstances of death.
Employment details

The employment details of the deceased worker at the time of death, including:

  • name of employer/s
  • address of employer/s
  • employment status (e.g. full-time, part-time, casual, contract)
  • occupation
  • usual hourly and weekly rate of pay including overtime.
Dependants

Details about the applicant and any known family members of the deceased worker who may have been reliant, to any extent, on the earnings for the worker at the time of death, including:

  • name, address and date of birth
  • relationship to the worker e.g. partner
  • the extent to which each person was reliant on the earnings of the worker at the time of the worker’s death.
Other documentation

Request the applicant to provide other information needed to consider Immediate Support payments. Types of documentation that may be useful include:

  • a marriage certificate or evidence of a registered relationship under the Relationships Act (2008)
  • a statutory declaration that confirms the relationship between the applicant and the deceased worker (if no marriage certificate or registration of the relationship under the Relationships Act exists)
  • employer statements, pay slips, contract of service
  • accounts/invoices for:
  • burial, cremation or repatriation
  • family counselling
  • medical and like services
  • copies of a Certificate of Title, joint bank statements, mortgage account/s, tenancy agreement or utilities or telephone bills that would confirm that the partner resided with the worker at the time of death.
Court-appointed trustee Request the trustee to provide a copy of Administrative Orders from court or VCAT which include the named administrator and their powers.
Contact a third party

The Agent may be able to verify or request information from an independent source, e.g. the employer about wage details or the funeral director about the burial/cremation account.
