5.5.6.4 130 Week vocational assessment report

The Agent approves a 130 Week Vocational Assessment OR service as part of a review to determine a worker’s entitlement to weekly payments post the second entitlement period (i.e. whether weekly payments should continue for the worker after 130 weeks of weekly payments has been paid).

A 130 Week Vocational Assessment Report will summarise the information collected by the OR Occupational Rehabilitation provider at the time the report was completed.

Refer to the WorkSafe Victoria Occupational Rehabilitation Fee Schedule for minimum report requirements.

Determining if you need a 130 week vocational assessment

As part of collecting appropriate evidence for this review, the Agent considers all OR information already obtained, including OES Original Employer Services reports, Transferable Skills Analysis and other NES New Employer Services reports. If this information provides sufficient evidence then the Agent may decide that a 130 Week Vocational Assessment Report may not be necessary.

A supplementary 130 week Vocational Assessment (RC505) may also be requested where other relevant information (for example NES reports) is on file and therefore a full RC500 130 Week Vocational Assessment is not required.

Approve vocational assessment report

When the Agent approves a RC500 - 130 Week Vocational Assessment Report, they must advise the worker in writing of the purpose of this type of vocational assessment and offer the worker a choice of OR provider if required.

Send OR approval form

When the Agent sends an OR Approval form to the OR provider, they include details about the type of information they expect to be included and nominate the fee that they have approved for the report.
