Incident Summary:
An electrician employed by Paper Australia Pty Ltd died as a result of being crushed between a lifting bucket mechanism and a reel of paper. The man was carrying out maintenance work when a contractor, who had not been trained in operating the lifting bucket, attempted to test the machine by lowering it. The electrician was caught between the bucket and the paper reel, and received fatal injuries. From where he stood at the operating panel, the contractor could not see the electrician.
The magistrate said the defendant had made an extensive and impressive plea and had acted in an exemplary manner after the incident. However, he also said that general and specific deterrence had to be considered.
